That's what it is. I bought it from a riverside bar - a shack with some crates of beer - in Burma about a dozen years ago. They wanted to give me a new one, but I wanted this, smooth and worn. It's a simple but effective design, and I had an idea to market a hi-tech version, did some drawings, spoke to a few people, but nothing came of it. It works by catching the screwhead under the cap, and pushing down on the handle. You can do it one-handed. Brilliant.

Late to the party as usual, but I would have guessed a piece of wood with a hole and a screw. That's what it is, what it is used for has already been elucidated.
As appealing as your "guess" is, it's wrong. It's a piece of wood with two holes, through one of which passes a bolt (not a screw), secured by two nuts.
The screwhead on the bolt is crucial, because the edge is sharp enough to catch under the lip of the cap. A bolthead will not work, nor will a screwhead that's too small. Also the distance between the screwhead and the wood - you can see from the photo how innumerable bottle caps have left their mark. This is teak, but you can use any hardwood.
